Thank you for your interest in the services at Julie Saad Wellness. I am here to provide you with Wellness Coaching guidance to support and improve your overall health and well-being.

By enrolling in coaching services, you as the client agree to all of the following:

I understand that a Wellness Coach does not diagnose or treat any disease, disorder or condition and is not a licensed naturopathic doctor, physician or dietician. 

The information collected in any intake forms and/or in intake and subsequent conversations regarding my health history, dietary habits and lifestyle will be used to get an overall picture of my health so Julie Saad can (i) safely recommend changes to improve my general health and well-being, (ii) educate me about the benefits of sound nutritional and other lifestyle choices and is not intended for use to prevent, diagnose or treat any existing condition or disease.

I represent and warrant that all medical information provided to me by Julie Saad during my initial health assessment is true and correct. I will advise Julie as soon as possible of any errors in the medical information I provided her or any changes to my current health status including any disease or ailments that I may be suffering from, any medication or over the counter drugs that I am consuming, allergies and whether I am pregnant, suspect I may be pregnant or am breast-feeding.

Should Julie Saad suspect I may have a disease, condition, or disorder, I will be informed of this suspicion and advised to consult my physician or naturopathic doctor for a proper diagnosis. By sharing this suspicion with me, I understand it is not a diagnosis or conclusion about the state of my health.

I hereby acknowledge and agree that Julie Saad Wellness has not made any warranties or representations of any kind to me regarding results that may be achieved from the services provided. I understand that any results or outcomes arising from these services are individual and may vary from person to person. I understand that the results of the services provided are not guaranteed and do not expect the Wellness Coach will be able to anticipate and explain all risks and complications. With this knowledge, I voluntarily consent to the suggested services provided to me by Julie Saad Wellness.

I hereby acknowledge and agree that the services may lead to certain unforeseen complications. I also acknowledge that I have the right to accept or reject any services suggested of my own free will and choice.

I recognize that specific foods, herbs and supplements may create allergic and possible serious reactions, in particular, products containing nuts. I further acknowledge that these allergies may cause me serious bodily harm. I represent and warrant that I have provided Julie Saad Wellness, during my initial health and assessment a list of all items which, to the best of my knowledge, I am allergic or sensitive to.

I understand that my identity will be protected at all times and, if necessary, identifying information will be altered to protect my privacy. I understand that a record may be kept of the services provided to me. This record will be kept confidential and will not be released to others unless so directed by myself or unless law requires it. Notwithstanding, I understand that Julie Saad Wellness is legally obligated to supersede confidentiality if they become aware of current child abuse or neglect, threats or harm to any individual and/or serious threat of suicide involved with my case. I understand that I may look at my records at anytime and can request a copy of it by paying the appropriate fees. 

In consideration for Julie Saad Wellness agreeing to provide me with the agreed upon services, I hereby irrevocably and unconditionally release and forever discharge Julie Saad and Julie Saad Wellness, its directors, officers, employees, agents and contractors and successors, assigns, heirs and legal representatives (all such persons and entities being called the “Released Parties”) of and from all manner of actions, causes of action, suits, demands, debts, accounts, covenants, contracts, damages and all other claims whatsoever, which the undersigned or its successors or assigns ever had, now has or may in the future have against any of the Released Parties for or by reason of any cause, matter or thing related to or arising from the services provided to me including (but not limited to) any bodily harm or injury sustained by me or any other person. I further acknowledge and agree that the Released Parties will in no way be responsible for any bodily harm or injury suffered by me as a result of the services.

I further agree and acknowledge that Julie Saad has the right to cease providing the services to me at any time effective immediately, without any compensation to me whatsoever. This letter of agreement will cover the entire course of my services and I am free to withdraw my consent and to discontinue participation of those services at any time. 

I understand that coaching is a comprehensive process that may involve all areas of my life, including work, finances, nutrition, health, relationships, education, and recreation.  I acknowledge that deciding how to handle these issues, incorporating coaching into those areas, and implementing my choices is exclusively my responsibility.  I understand that coaching is not to be used as a substitute for professional advice by legal, financial, or other qualified professionals and will seek independent professional guidance for such matters.

I understand that coaching does not involve the diagnosis or medical treatment of mental disorders as defined by the American Psychiatric Association.  I understand that Coaching is not a substitute for counseling, psychotherapy, psychoanalysis, mental health care, or substance abuse treatment, and I will not use it in place of any form of diagnosis, treatment, or therapy.

CANCELLATION TERMS Please note Julie Saad Wellness requires 24 hours' notice for rescheduling of appointments or the full visit fee will be charged.

30-DAY MONEY BACK GUARANTEE I understand that I can request and receive a full refund within 30 days of the sign-up date for any reason, and that requests for refunds after that date will be denied.

I hereby certify that the representations and warranties contained in this letter agreement are true and correct and I hereby agree to the foregoing covenants and terms and conditions.

By enrolling in this program, I hereby grant consent for Julie Saad / Julie Saad Wellness to provide me with the services described above.
